Business Associate Agreements

Where required by HIPAA, CaseWise Nurse Consultants will enter into a Business Associate Agreement (BAA) with covered entities and their legal representatives prior to receiving or reviewing any PHI. Please contact us to request a BAA before transmitting any patient records or identifying health information.

No PHI via Contact Form

The contact form on this website is not a HIPAA-compliant transmission channel. Do not submit patient names, dates of birth, Social Security numbers, medical record numbers, or any other PHI through the contact form. All case materials containing PHI must be transmitted through the secure methods established at the outset of our engagement.

Upon conclusion of an engagement, case materials are returned, destroyed, or retained in accordance with the terms agreed upon with the retaining attorney.
